Institutions must follow the Model Privacy Form instructions when using the Online Form Builder.
- If you provide an opt out and want to include affiliate marketing, use Form 1 (based on version 2 & its instructions).
- If you provide an opt out and do not want to include affiliate marketing, use Form 2 (based on version 2 & its instructions).
- If you do not provide an opt out and want to include affiliate marketing, use Form 3 (based on version 1 & its instructions).
- If you do not provide an opt out and do not want to include affiliate marketing, use Form 4 (based on version 1 & its instructions).
- If you provide an opt out, you want to include affiliate marketing, and have a mail-back form, use Form 5 (based on versions 3 & 4 and their instructions).
- If you provide an opt out, you do not want to include affiliate marketing, & you have a mail-back form, use Form 6 (based on versions 3 & 4 and their instructions).
