- This form is revised to clarify to use the name change only with adoption, change of marital status, or court order – not for adding, removing, or substituting owners. It has added space for an additional owner; space to note State/Issue/Expiration of D.L. or state ID, spaces for mobile phone and email address. The following items have been removed from the form: Social Security Number and the instructions about whether all owners must sign to authorize changes. The latter issue should be set by CU procedures, and may vary depending on the circumstances.
