Use for: Regular personal accounts, custodial accounts under UTMA, and business/organization accounts under Wisconsin law. This form provides additional terms and conditions beyond those included in the various account ownership agreements offered by The League. Another form, Your Account Agreement #83006, is available for use by credit unions that want the account agreement terms and conditions and an arbitration agreement with class action and jury trial waivers.
The credit union must supplement this form with additional disclosures as may be appropriate for the account.
Use with Account Ownership Agreement, Custodial Account Ownership Agreement, and Organization Account Ownership Agreement revised 6/21 or later.
