Standardized terms for member accounts. Does not include account disclosures. Do not use if incompatible for CU policies and procedures.
Use for: Regular personal accounts, custodial accounts under UTMA, and business/organization accounts under Wisconsin law. This form provides additional terms and conditions beyond those included in the various account ownership agreements offered by The League. It also includes an arbitration agreement, with class action and jury trial waivers. Another form, Your Account Agreement #83006, is available for use by credit unions that want the account agreement terms and conditions but without the arbitration agreement. The arbitration agreement provides for an accountholder’s right to opt out of the arbitration clause for 30 days they receive the agreement.
The credit union must supplement this form with additional disclosures as may be appropriate for the account.
